Documentation Guidelines For Learning Disabilities
It is the policy and practice of UW-Stevens Point to comply with Section 504 of the Rehabilitation Act as well as with the Americans with Disabilities Act. Accommodations appropriate to the disability shall be provided to qualified individuals who provide documentation to Disability Services.
Documentation for Learning Disabilities shall include:
- A clear statement of the diagnosis from a qualified authority (Psychologist, Psychiatrist, Learning Disabilities Specialist.)
- A clinical interview description of the testing procedures, instruments used and test and subtest scores. Interpretation and recommendations based on the data gathered is recommended.
- Test results in the following areas: intelligence, reading, math, spelling, written language, and cognitive processing.
- A clear diagnostic statement based on the test results.
- The following instruments are examples of tests that would be considered for use in a diagnosis of a learning disability:
- Wechsler Adult Intelligence Scale-Third Edition (WAIS-III)
- Woodcock Johnson Psycho educational Battery III, Cognitive & Achievement Sections
- Wechsler Individual Achievement Test (WIAT)
- Woodcock Reading Mastery Test (WRMT-R)
- Key Math Test
- Other tests may be deemed appropriate based on the expertise of the clinician and the presenting issues of the individual tested.
