Assignments for the Week-Outcomes
- Take first quiz in D2L
(Open this week from Monday, January 30 @ 12:01 a.m. until Sunday,
February 12 @ midnight). The quiz will cover concepts from our first
class session. (Note: The first quiz is open for two weeks; all
others are open only for one)
- Go to the Testwell site by
clicking on the link below. Enter your student i.d. number, when
asked for a pin number, respond to all
the questions, and click "finish" when you're finished and view your results. You may want to print them
for a record that you completed the assignment. We will receive a notice that you've completed Testwell and post your points within two weeks of the closing period.
Complete Testwell for full points by NOON. on Monday, February 20, 2012. This assignment
will only be available for completion (it is considered late and worth half
the points after February 20) until February 27, 2011. After
this date you cannot complete this assignment for any points. Keep a
record (printout) of the results page (last page) in case of technology glitch to ensure
proper credit. We will receive a list of all student i.d. numbers who
have completed the survey after the closing date and you will receive points
at that time.

4. Complete the
Fitness Medical Release Form.
Follow any special
instructions given to you on the results page. Be sure to enter your
full name and student ID number. Print off a copy of the completed form
and sign it. EVERYONE bring your form
to class next week.
If you answer “Yes” to any question on the form, you
MUST be cleared by Health Services staff in class. If you
forget to bring the form to class, you will have to make an appointment at
the Health Center in Delzell Hall and take it to them to sign.
Other things to do for next week:
- Locate THE BOX in CPS (across the hall from CPS 210) for future deposits
of assignments.
- Bring crayons or markers to class next week!
- Start reading Chapter 1 (An Invitation to Healthy Change).
